Print and fill in this Tyson Any’tizers form. Purchase 4 bags starting Monday 1-02 through 2-17-12. Mail with the 4 proofs-of-purchase (I assume they mean UPC codes cut from the package) along with your dated cash register receipt with the prices circled. Mail on or before 3-16-12. You will receive by mail a $10.00 sporting goods gift certificate good at participating retailers. Think Super Bowl parties and look for sales and coupons before buying!
